How should I go about this with my Dr.?

I have TMJ and its been bothering me again. I need to get some pain reliever from my Doctor for the TMJ pain. I am also leaving very shortly for a vacation and I tend to have panic attacks when I fly. I live in a new city and would need to find a new doctor that doesn't know my history with these problems. I am worried if I ask for both these highly addictive prescriptions he will think I'm just trying to score some pills.... What do you think? Any similar situations?
